NY Direct Accounts - Signed ST120 and Customer Application document required (Blank ST120 available for download on this page)
CA Direct Accounts - Signed CA Resale Certificate with T. Elenteny Imports or TEI listed as the Vendor is required
IL Direct Accounts - Copy of current IL Liquor License is required
